Terms and Conditions

These Terms and Conditions (“Agreement”) outline the rules and responsibilities governing the use of services provided by Covenant Home Care (“Company”, “We”, “Us”, or “Our”) and the obligations of the client (“Client”, “You”, or “Your”). By engaging our services, you agree to be bound by the terms outlined in this Agreement.

 

 

1. Services Provided

 

Covenant Home Care offers non-medical, in-home care services, including but not limited to:

• Personal care assistance (bathing, grooming, toileting)

• Companionship and supervision

• Light housekeeping

• Meal preparation

• Medication reminders

All services will be detailed in a personalized care plan agreed upon prior to the commencement of service.

 

 

2. Client Responsibilities

• Access and Safety: The client must provide a safe environment for our caregivers. Unsafe or abusive conditions may result in suspension or termination of services.

• Accurate Information: You must provide accurate medical and personal information necessary for effective care.

• Payment: The client is responsible for timely payment as outlined in the Service Agreement or invoice.

 

 

3. Scheduling and Cancellations

• Changes in Schedule: We require at least 24 hours’ notice for any cancellation or schedule change.

• Cancellation Fees: Less than 24-hour notice may incur a cancellation fee equivalent to one (1) hour of service unless due to an emergency.

• Holiday Rates: Services requested on recognized holidays may incur additional charges.

 

 

4. Payment Terms

• Invoices are issued bi-weekly (or as agreed) and are payable within 7 days of receipt.

• Accepted payment methods include check, credit card, and ACH bank transfers.

• Overdue payments may result in service interruption and a late fee of 5% per unpaid invoice.

 

 

5. Caregiver Conduct and Replacement

• All caregivers are vetted, trained, and employed or contracted by Covenant Home Care.

• If a caregiver is not a good fit, we will provide a replacement upon request.

• Clients may not solicit or hire our caregivers independently for private work for up to 12 months after termination of services.

 

 

6. Confidentiality

 

We respect the privacy of our clients. All client information is treated as confidential and will not be shared without consent, except as required by law or in case of medical emergencies.

 

 

7. Limitation of Liability

 

While we strive to provide the highest quality care, Covenant Home Care shall not be held liable for:

• Any injury or loss caused by client noncompliance with care plans.

• Damages resulting from acts beyond our control, including force majeure events.

• Any incidental or consequential damages arising from the use of our services.

 

 

8. Termination of Services

 

This agreement may be terminated:

• By the client with 48 hours’ written notice.

• By Covenant Home Care with cause (e.g., unsafe environment, non-payment) or without cause with 7 days’ notice.

 

 

9. Amendments

 

Covenant Home Care reserves the right to modify these terms at any time. Updated terms will be communicated to clients and posted on our official website or shared by email.

 

 

10. Governing Law

 

This Agreement shall be governed and construed in accordance with the laws of the state of Alabama, without regard to its conflict of law principles.

 

 

Acknowledgment

By using Covenant Home Care’s services, you confirm that you have read, understood, and agreed to the Terms and Conditions set forth in this Agreement.
